What is a superbill?

A superbill is an itemized statement of healthcare services. In pediatric private practice, families often need one to seek out-of-network reimbursement or keep HSA/FSA records — even when you are not billing insurance directly.

What does a therapy superbill include?

  • Rendering provider name, credentials, NPI, and tax ID
  • Practice address and place of service code
  • Patient name and date of birth
  • Dates of service with CPT codes, units, and fees
  • ICD-10 diagnosis codes
  • Total amount and signature when required

How to create one (in four steps)

  1. Step 1

    Gather provider and patient details

    Name, credentials, NPI, tax ID, address, patient name and date of birth.

  2. Step 2

    List each date of service

    Add CPT code, units or duration, and fee for every visit on the document.

  3. Step 3

    Add diagnosis codes

    Include ICD-10 codes that support medical necessity for the services billed.

Is a superbill the same as a claim?

No. A claim is submitted to a payer (often electronically). A superbill is a detailed receipt families or providers use to support reimbursement or records.

Who creates the superbill?

Usually the treating clinician or their practice. Parents may request one for out-of-network benefits.
